Pearl Health Clinic (PHC) is a large outpatient multidisciplinary community mental health clinic that commits to each patient by giving them unparalleled treatment that uncovers the worth we all have as human beings. We are seeking a Community Based Rehabilitation Specialist, Case Manager, TCC to improve the quality of life for people recovering from a severe and persistent mental illness. The Community Based Rehabilitation Specialist meets one on one with patients on a regular basis and follows a carefully developed treatment plan in order to help clients build functional behaviors and coping skills for interaction with their surroundings. The Community Based Rehabilitation Specialist aids patients in gaining and utilizing skills necessary to function adaptively in home and community settings and attain or retain capability for independence.

- Uphold and implement all state regulations according to OPTUM
- Attend all staff trainings and meetings as required or requested
- Meet one on one with clients on a regular basis
- Participate in client assessments/reassessments
- Assist client in development of goals, objectives, and reviews as appropriate
- Have any necessary material for program objectives available throughout therapy sessions
- Maintain the safety, health, and wellbeing of clients at all times
- Be in continuous, respectful, and appropriate contact with clients’ guardians, siblings, teachers, etc. as applicable
- Report concerns, incidents, accidents, findings, etc. to the CBRS Manager, Administrator, and/or Child Protective Services as needed
- Keep client files current and organized
- Complete all clinical documentation in the EMR in a timely and accurate manner
- Be on time to pick up and drop off clients, using time responsibly and to the fullest extent
- Follow a carefully developed treatment plan in order to help clients build functional behaviors and coping skills for interaction with their surroundings

- Completion of the PRA CPRP or CFRP certification after 30 months of providing CBRS services.
- Bachelor’s Degree, in Social Work, Psychology or closely related to human services is preferred.
- Be able to successfully complete drug screening, background check, and I-9
- Have a current Idaho driver’s license and ability to meet insurance requirements for personal vehicle
- Experience preferred but not required for the right candidate.

Direct Support Staff
Job Summary
Assists the Program Coordinator, Residential Habilitation Administrator, and Staff Manager in forming and supervising the Residential Habilitation program to provide a learning environment ensuring individual receiving services achieve their maximum possible individual growth and the highest level of community integration. Provides training to the individual receiving services to increase adaptive skills. Assist in the implementation of individual support plans and the implementation plan. Supervises and assists individual receiving services in activities of daily living. Assist in maintaining a safe and sanitary environment.
Responsibilities
- Resolves conflicts between individuals receiving services
- Assure that you are using the appropriate techniques, training the individual receiving services in individual and group settings designed to promote growth and achieve independence, including:
- Behavioral management: reinforcing appropriate responses and implementing natural consequences to mal-adaptive behaviors
- Self-help skills: eating, dressing, grooming, health, maintenance, and toileting
- Communication skills: receptive and expressive
- Motor skills: object reception, object projection, and body projection.
- Social/recreational skills: socially appropriate behavior and recreational abilities
- Community living skills: orientation, travel, and money concepts
- Domestic skills: housekeeping and care of personal belongings
- Medication skills: assists consumer to recognize medication and safely take the medication(s) at the appropriate time
- Assists in the formulation and implementation of Individual Support Plans, including:
- Collecting data on clients and writing summaries each day for use in programmatic decisions
- Provide written reports to the ISP Team, consultants, and others by completing the paperwork as required or when directed
- Carrying out ISP recommendations
- Checks and assures all documentation is accurate and completed in a timely manner, including, but not limited to:

- Assists in the coordination, supervision, and implementation activities and outings
- Assure individuals receiving services are supervised in activities of daily living including:
- Maintaining all hygiene including showering, brushing teeth, grooming
- Eating, serving meals, preparation of meals, appropriate table manners, and cleanup
- Dressing—appropriate to place and weather, clean and neat in appearance, and appropriate clothing care
- Assures individual receiving services is provided with appropriate transportation
- Assists in maintaining a safe and sanitary environment, including:
- Protecting individual receiving services from physical, verbal, or mental abuse
- Awareness of individual receiving services’ whereabouts at all times
- Notifying appropriate staff of safety hazards
- Being familiar with emergency evacuation procedures
- Being familiar with emergency first aid procedures and assists medical staff as necessary
- Maintains trainer certification as outlined in staff development procedures, including:
- Attendance at required in-services training and staff meetings
- Maintains quality and quantity of training
- Successful completion of required course work and training proficiency checks
- Obtains and maintains a working knowledge of the Residential Habilitation regulations
- Listens, acknowledges, and makes decisions based on staff complaints or individuals receiving services and reports them to Administrator, Program Coordinator, or Staff Manager
- Models appropriate behavior to other staff and individual receiving services
- Receives and maintains training in Assisting with medications certificate, to include
- Make consistent and accurate medication passes
- Follow all policy and procedures for medication passes
- Attend any in-services or staff meetings regarding medication passes
- Communicates daily at change of shift to assure communication concerning individual receiving services’ moods, behaviors, appointments, health, or any other significant information
- Assures activity schedules are being implemented
- Assists in scheduling and maintaining proper staff/ participant ratio
- Responsible for notifying supervisors of behaviors, unknown injury (Administrator notification), participant to participant contact (Administrator notification), abuse, mistreatment, and neglect (Administrator notification)
- Communicates professionally and effectively with all community, state, and federal organizations or business contacts involved with the Residential Habilitation
- Ensures all individual receiving services attend scheduled meetings within the community
- Treats all employees and individual receiving services with dignity and respect
- Any other reasonable duty assigned to you either verbally or in writing by the Program Coordinator, Administrator, Staff Manager, House Supervisor, or House Coordinator

Psychologist
Position Summary
Doctoral level mental health care provider who conducts clinical interviews with clients to determine treatment needs and appropriate services. Creates a strong therapeutic alliance with clients. Partners with clients, providing psychoeducation and psychotherapy, as they work toward personal goals. Conduct Psychological and Neuropsychological Testing to inform diagnosis and treatment planning. Provides clinical supervision for students.

Practice
Pearl Health Clinic (PHC) is a large outpatient multidisciplinary community mental health clinic serving all ages and presenting problems throughout Eastern Idaho. Services provided include clinical interviews, outpatient psychotherapy, medication management, neuropsychological and psychological testing. There are over 20 licensed behavioral health providers practicing at Pearl Health Clinic.
Location
Pearl Health Clinic is located in Ammon, a city within the Idaho Falls Metropolitan area of approximately 150,000 residents. Idaho Falls is Idaho’s largest city outside the Boise metropolitan area. Idaho Falls offers an abundance of shopping, restaurants and entertainment opportunities. The city has nearby golfing, skiing, ice-skating, an aquatic center, aquarium, and zoo. It has a beautiful river walk trail known as the Greenbelt and is a short day trip away from many different outdoor activities and attractions such as Jackson Wyoming.
